New Patients
This page is intended to help familiarize new patients with what to expect when they come for physiotherapy for the first time.
I strongly recommend that you read the FAQ section.
Also, it will help me tremendously and speed up your assessment if you were to complete, as fully as possible, my Patient Forms in advance of your assessment. Completing the forms can take as long as half an hour, and I would rather spend that time assessing you.
The forms can be filled out on your computer and you can print them out and bring them with you on your first visit.
Clicking on the link to the form will cause a small form filling program to download to your computer automatically. Just click on "save" and you can download the form to your desktop. Once download on your desktop, double click the form icon on your desktop and a window to OmniForm Mailable Filler will open with the form. You can complete the form on your screen, print it and bring it back to me on your next visit or email it to me.
